How to remember lead placement?
- ▷ 12 Lead Placement guide with diagram
To remember ECG lead placement, use the "Smoke over Fire" and "Snow over Grass" mnemonics for limb leads (black over red, white over green), and remember "Brown" for V1 ("Chocolate on the Stomach," i.e., right sternal border, 4th ICS) for precordial leads. The chest leads (V2-V6) are placed sequentially down the rib cage at specific landmarks, which you can remember using a color-coding mnemonic like "Rainbows yield great beauty offering peace".
Limb Leads
Right Arm (RA) & Left Arm (LA): Black lead (LA) goes above the red lead (RA), which is referred to as "Smoke over Fire".
Right Leg (RL) & Left Leg (LL): White lead (RA) goes above the green lead (RL) for "Snow over Grass".
White on Right: Remember that the white lead (RA) is placed on the right arm.
Chest (Precordial) Leads
V1:
Brown lead (V1) is placed at the fourth intercostal space (ICS) on the right sternal border.
V2:
Yellow lead is placed at the fourth intercostal space on the left of the sternum.
V3:
Green lead is placed midway between V2 and V4.
V4:
Blue lead is placed in the fifth intercostal space at the midclavicular line.
V5:
Orange lead is placed in the fifth intercostal space at the anterior axillary line.
V6:
Purple lead is placed in the fifth intercostal space at the midaxillary line.
